Here respondents were asked the question “how poor does a person’s general health have to be for it to become a barrier to a person deciding to get paid employment?”  The rating scale used was a 5-point scale ranging from 0 to 4.
0 = very poor
1 = poor
2 = fair
3 = good
4 = very good

If a respondent believed that a person’s general health had to be very poor for it to become a barrier to a person deciding to get paid employment they would give a score of 0.  If they believed it had to be poor they would give a score of 1 and so on.
